2011 Edition Changes

There are two changes in the dental coverage portion of your Welfare Fund benefits for 2011. Both represent small improvements in the benefit. Previously, you were entitled to a maximum of one implant per year and two per jaw in your lifetime. Going forward the reference to "two per jaw" has been removed, so you are covered for one implant per year up to a maximum of four in a lifetime, anywhere in your mouth. The trustees also have increased the number of times a member or dependent can receive a periodontal "deep cleaning" in a given year from four to six.

The PICA Drug Program

For those of you who utilize drugs in the PICA program, there are changes to the program as of July, 2005. Below we provide a link to the City's website with information about these changes. (A reminder: Medicare-eligible retirees are not covered by the PICA program.)

To determine your current Dental Benefit eligibility or locate a provider on our plan, you may click on the MetroDent icon to the left. Your browser will open a new window to display the website of Self-Insured Dental Services. Select OSA in the drop-down box that contains a list of unions and organizations served by SIDS and provide the information they request to identify yourself as a member. You will then be able to search a list of providers by geographic area and read plan descriptions.

To determine your current optical benefit eligibility or locate a provider on our plan, you may click on the DavisVision icon to the left. Your browser will open a new window which will display the DavisVision website. If you wish to login as a member click on "Member Login." If you wish to find a participating provider click on "Find A Doctor." You will be asked for personal identifiers (your social security number and a password).
